A transition is a short effect placed on the cut where two clips meet. Premiere Pro's own transitions live in the Effects panel, and adding one takes seconds. The real skill is putting it in the right place at the right length.

Put the playhead on the cut, or select the cut, and press Cmd/Ctrl+D; the default video transition is applied. For audio it is Cmd/Ctrl+Shift+D. To change the default, right-click a transition in the Effects panel and choose Set Selected as Default Transition.
A transition uses frames from both clips beyond the cut point. If a clip was cut at the very start or end of its source, those frames do not exist; Premiere warns you and repeats the last frame. The fix is to trim a few frames off the clip ends and join the cut again.

A transition on every cut wears the viewer out; a transition means something when the scene or topic changes. Keep them short in fast content: most work better under half a second. Sticking to one or two kinds within a video keeps the edit tidy.
